It is the cover page of the record, and it tells you who you are reading about and why they are in hospital before you touch the narrative notes.

Reviewing a survey taught me to spot self-selection bias and narrow sampling, and to remember that statistically significant does not mean important. Now the first question is always what the evidence is, and whether the method supports the conclusion.
